If you are receiving services from Connections, we would like your suggestions and feedback about how we can improve our programs. We want to be sure that you are receiving the services you need and want, and that you are happy with the way in which you are being served.

Also, if you have a family member who is receiving services from Connections and you have suggestions and feedback about how we can improve our programs, we welcome your feedback and suggestions as well.
