Connections has been a pioneer in the delivery of housing and community-based services for individuals and families who experience homelessness in Delaware, operating more than 500 units of housing for households with incomes below 30 percent of the area median.

Most of the housing we provide was developed as part of the Low Income Housing Tax Credit program, or one of several HUD programs (Continuum of Care for the Homeless Supportive Housing Program, 811 program for individuals with disabilities, or the Housing for People with Aids (HOPWA) program).

We partner with many private and public agencies to provide supportive services to our residents in permanent housing, using leveraged and mainstream resources and our status as a provider of primary medical and behavioral health services for Medicaid and Medicare.

We ensure that all program participants have access to the necessary resources including medical, psychiatric, substance abuse, case management and employment services.

To obtain housing assistance through Connections, prospective clients must participate in Delaware’s Centralized Intake process. To contact intake, call 302-654-0126.
